Metoidioplasty, sometimes referred to as a meto or meta or spelled "metaoidioplasty", is an alternative to phalloplasty for trans men. With the effects of testosterone treatment, the clitoris enlarges, over time, to an average of 4-5 cm (due to the clitoris and penis being developmentally homologous organs).

In a metoidioplasty the enlarged clitoral tissue is released from its position and moved forward to more closely approximate the position of a penis.

Metoidioplasty is technically simpler than a phalloplasty, and has fewer complications. The surgery itself is also considerably shorter (2-3 hours vs. 8-10 hours overall) and it is much less expensive. Unlike a phalloplasty, an erectile prosthesis is not needed to achieve erection, although the erection is usually less hard compared to the penis of a cisgender man. Because metoidioplasty can not achieve penile dimensions comparable to a cisgender penis, the neopenis is usually not capable of penetrative sexual intercourse. Genital orgasm is almost always retained after surgery (unlike phalloplasty which carries significant risk of loss of erogenous sensation).

Mandibular Angle Reduction (Reduction Angleplasty) is done to individuals who have wide jaw line or square face. It reduces the width of the lower face and change a square face to an oval one.

Duration of the operation:

The procedure is performed about 1-2 hours.

Post operative care:

Swelling and skin discoloration are common and usually will subside within a week or two. Stitches will be removed after about seven days. When the operation is carried out from the inside of mouth, gargle your mouth with mouthwash for at least 10 days. Keep incision region clean and dry. Apply prescribed antibiotic ointment to staples and stitches. Dry thoroughly with gauze. Keep ice compresses on the surgical region continuously for the first 48 hours. Do not use hot dryer, when dry up your hair. Avoid steam baths and saunas for several months. Start with clear fluids and increase as tolerated. Avoid foods that are chewy for the first 2 weeks. No chewing of hard foods or tough foods for 2-4 weeks after surgery. You are to stay on a light diet. No alcohol and no smoking for 2 weeks if possible as they increase swelling; or have only in moderation. You will be asked to keep your head slightly elevated for a few days. Rest quietly in bed with head raised on 2 - 3 pillows. During the first 48 hours bed rest is recommended. During the next 3 to 5 days, you should restrict your normal activities, no housework and no physical labor. During the first 5 days after surgery, it is most important not to engage in any activity which will elevate the blood pressure and possibly increase bruising, swelling and bleeding. Walking is permitted as tolerable. Do not lie face down for one month. When bending down, bend at knees, not with head down.

Recovery period:

Expect to feel tired for 7 - 10 days after surgery, especially after anesthetics. Don't rub or scratch surgical region for at least 2 weeks. Wound dressing should be done twice a day for 7 days, after tape is removed. Take antibiotic for 14 days after surgery. Do not take any vitamin E and aspirin (or acetyl salicylic acid) for 14 days after surgery, because they can increase your tendency to bleed and prescription pain medications will be given for pain relief.

Possible risk and complications:

All surgical procedures carry some risk. Hair transplant is a procedure in which healthy hair is taken from the back or sides of the head (donor site), and placed in the balding area on the scalp. Duration of the operation:

Treatment up to 1,000 grafts may take 3-6 hours. Treatment of more than 1,000 grafts will normally be done over 2 or 3 days.

Post operative care:

Hair transplantation is a very safe, relatively minor surgical procedure. Patients are provided with a mild analgesic to relieve any discomfort felt the night following the procedure. Small crusts may form on each graft but you can comb over this area to cover these scabs. These crusts will flake-off by 10-14 days after the surgery. The transplanted hair seen above the scalp will initially be shed, however the roots will remain dormant for 6-12 weeks, at which time the new hairs will all begin to grow. Numbness that may occur in the donor or recipient area usually disappears in 2-8 months following surgery. Patients may experience mild swelling to the forehead area for a few days following surgery.

Recovery period:

The treated area may have slight swelling for a couple of days. Scabs form and fall off within 2 weeks. The skin of the recipient area is healed within 2 weeks. Some or all of hairs in the transplanted grafts drop out in 2-8 weeks. The skin of the donor area is healed within 2 weeks.

Complications are rare. Severe infection is highly unusual due to the abundant blood supply in the scalp, but minor infections can occur around a newly transplanted hair follicle, similar to an infected ingrown hair or pimple, and responds easily to antibiotics. Minor scars that occur in the donor scalp as a result of the removal of donor skin are narrow and can be easily hidden by the surrounding hair. The graft sites in the recipient frontal area heal with almost no visible scarring and are covered by the transplanted hair. Patients may experience mild swelling to the forehead area for a few days following surgery. In rare occasion, parts of the scalp can remain numb after each procedure but this usually disappears in 2-8 months after the surgery
